Physiotherapy Training in Germany for Indian Students

Physiotherapy training in Germany sits outside the dual Ausbildung system most of this site covers — it runs under different legislation, follows a different pay structure, and asks for a different level of German. If physiotherapy is the specific path you’re considering, here’s what actually applies.

Why this one is structured differently

Physiotherapy is regulated by its own federal law, the Masseur- und Physiotherapeutengesetz (MPhG) — not the Berufsbildungsgesetz (BBiG) that governs the dual, company-based trades covered elsewhere on this site. That single difference explains most of what’s unusual about it: training happens at a state-recognised Berufsfachschule (vocational school) rather than primarily inside a company, and it isn’t automatically paid the way BBiG apprenticeships are.

What the role involves day to day

Physiotherapists assess movement and physical function, then design and deliver treatment for injury, chronic pain, post-surgical recovery, and mobility conditions — manual therapy, therapeutic exercise, and rehabilitation planning, working with patients individually across hospitals, private practices, rehab clinics, and sports medicine settings. It’s a clinical, hands-on role with direct, ongoing patient contact from early in training.

Duration and structure

Training runs 3 years under the MPhG, made up of at least 2,900 hours of theoretical and practical instruction plus 1,600 hours of supervised practical training in clinical settings, finishing with a state examination (staatliche Prüfung). The law also allows shortened routes for people with relevant prior qualifications or professional experience, but those are exceptions, not the standard path for a new entrant.

German language requirement — and it’s higher than the usual document route

The general B1 document route that applies to most Ausbildung applications isn’t the full picture here. Germany’s official recognition portal, Anerkennung in Deutschland, states that to actually hold the professional title and licence to practise (Berufszulassung) as a physiotherapist, you need subject-specific German at B2 level on the Common European Framework, since physiotherapists communicate directly and continually with patients, doctors, and colleagues. Several federal states add a further profession-specific language test on top of the general B2 requirement. In practice, most training schools look for a credible route to B2 well before graduation, even if your entry paperwork only requires B1.

Pay during training — the part most guides gloss over

Because physiotherapy training sits outside the BBiG/tariff system that funds most Ausbildungsvergütungen, there is no guaranteed, legally set training wage the way there is for a dual apprenticeship. Training at a private Berufsfachschule can carry tuition fees (Schulgeld) that vary by school; state-run schools are typically free of tuition but may still not pay a wage. The one meaningful exception: trainees at publicly run providers — a municipal or university hospital (Klinikum), for instance — are sometimes paid under that institution’s own collective agreement. There’s no single verified average figure we can responsibly quote here, and be wary of any source that states one confidently — always confirm pay and fees directly with the specific school before enrolling.

Demand: what the data actually shows

The Bundesagentur für Arbeit’s Fachkräfteengpassanalyse — the federal shortage-occupation analysis covering roughly 1,200 occupations — names care and health broadly, alongside medical occupations, as one of its strongest shortage categories nationally. Physiotherapy sits within that broader medical-occupations grouping in BA’s own public summaries, though BA’s press materials don’t break out a physiotherapy-specific vacancy ratio the way they do for nursing. Treat the demand signal as directionally positive rather than a precisely quantified shortage for this exact title.

Is it the right fit?

This path suits people set on clinical, patient-facing healthcare work who are comfortable with a school-led (not company-led) structure, a longer runway to B2 German, and a training cost picture that needs checking school-by-school rather than assumed. If a paid, company-based dual Ausbildung with a clearer, tariff-set wage matters more to you than this specific profession, our Nursing & Healthcare Ausbildung programme page covers Pflegefachmann/-frau training, which is dual-structured and centrally financed — see our piece on elderly and generalist nursing care Ausbildung for how that compares.
